Pareto Care is technology enabled healthcare services company focused towards reducing Congestive Heart Failure (CHF) hospitalisations and readmissions. We are building a wearable biosensor platform to monitor patient hemodynamic parameters important in managing CHF treatment. Our solution includes a three stage process; monitoring, predicting and intervening. We monitor clinical parameters that detect early deterioration; using this data we predict CHF exacerbations and then work with clinicians to manage treatment in order to avoid hospitalisation. Our wearable biosensor platform includes two wearable medical devices; a wrist watch and a chest patch. Together these devices continuously, non-invasively and remotely monitor 8 parameters including Arterial Blood Pressure, Cardiac Output, Stroke Volume, and Thoracic Fluid Content. Our clinician interfacing web application allows clinician&s;s to continuously and remotely monitor patient hemodynamics, recieve customized alerts and exacerbation predictions, and recommendations for appropriate evidence-based interventions.
